On May 26, 2022, the five-year transitional period (announced in April 2017) for the new EU Regulation on In Vitro Diagnostic Medical Devices (IVDR 2017/746) has ended. This requires a transition from EU IVD Directive 98/79/EC (IVDD) to IVDR for all Class A products including nucleic acid isolation kits and non-sterile sample collection devices. The completion will be documented by the signing of a Declaration of Conformity (DoC). Our current IVD PAXgene Blood RNA Tubes are with sterile conditions and could still be sold until May 26, 2027 under their DoCs pursuant to the Directive 98/79/EC (IVDD). PreAnalytiX nevertheless intends to finalize the PAXgene Blood RNA Tubes IVDR transitions already within the upcoming months for supporting our customers early in their own IVDR compliance efforts. The required “EU Quality Management Certificate IVDR for Sample Collection Devices with Sterile Conditions” was recently issued by the EU Notified Body TÜV SÜD. Once the DoC has been signed, it may take several months for the IVDR product to be commercially available due to international regulatory product registrations outside of the European Union.

Product Details

The PAXgene Blood RNA System consists of a blood collection tube (PAXgene Blood RNA Tube) and a nucleic acid purification kit (PAXgene Blood RNA Kit). It is intended for the collection, storage and transport of blood and the stabilization of intracellular RNA in a closed tube, and subsequent isolation and purification of intracellular RNA from whole blood for RT-PCR used in molecular diagnostic testing.

Performance characteristics for the PAXgene Blood RNA System have only been established with FOS* and IL1B** gene transcripts. The user is responsible for establishing appropriate PAXgene Blood RNA System performance characteristics for other target transcripts.

*FOS is the gene symbol for human Fos Proto-Oncogene, AP-1 Transcription Factor Subunit.

**IL1B is the gene symbol for human Interleukin 1 Beta.

Performance

The additive in PAXgene Blood RNA Tubes protects RNA molecules from degradation by RNases and other enzymes and triggers the complete lysis of all cells in the sample to minimize ex vivo changes in gene expression due to gene induction and downregulation. The result is immediate and allows long-term stabilization of RNA in specimens for up to 3 days at room temperature (see figures " RNA stability at 18–25°C: FOS" and " RNA stability at 18–25°C: IL1B") or refrigerated for up to 5 days (see figures " RNA stability at 2–8°C: FOS" and " RNA stability at 2–8°C: IL1B"). Current data shows stabilization of intracellular RNA for up to 11 years at –20°C or –70°C (see Technical Notes).

Principle

The collection of whole blood is the first step in many molecular assays used to study intracellular RNA. A major challenge in this type of testing is the instability of intracellular RNA, which rapidly degrades within hours after blood collection. Furthermore, certain transcript levels change after blood collection due to ex vivo gene regulation, such as induction and repression. Both RNA degradation and gene regulation can lead to an under- or overestimation of in vivo gene transcript levels.

The PAXgene Blood RNA Tube contains an additive that stabilizes the in vivo gene expression profile by reducing post-collection RNA degradation and minimizing gene induction and repression. When used in conjunction with one of the PAXgene Blood RNA Kits, the PAXgene Blood RNA Tube provides pure RNA for accurate detection and quantification of gene expression.

Procedure

Whole blood is collected directly into the PAXgene Blood RNA Tube, using standard phlebotomy practice and equipment. The RNA in the specimen is immediately stabilized for storage at room temperature, refrigerated, or frozen. RNA can be purified using one of the PAXgene Blood RNA Kits (FDA and CE). The ease of collection, stabilization of RNA at different temperatures, and elimination of additional processing steps, such as centrifugation at the collection facility and storage on ice prior to RNA purification, contributes to standardize the workflow from collection to purification, reducing variability that can be introduced by different operators (see " Reproducibility between users.")

When used in conjunction with the PAXgene Blood RNA Kit, the PAXgene Blood RNA Tubes enable purification of intracellular RNA from whole blood for downstream applications in molecular diagnostic testing, such as real-time RT-PCR analysis of specific transcripts.

What are the expected yields of RNA from PAXgene Blood RNA Tubes using the QIAsymphony PAXgene Blood RNA Kit?
RNA yields isolated from 2.5 ml healthy, human, whole blood (4.8 x 106 –1.1 x 107 leukocytes/ml) are specified to ≥3 µg for >95% of the samples processed (see Technical Note for the QIAsymphony PAXgene Blood RNA System ). Since yields are highly donor-dependent, individual yields may vary. Mean yields in the range of 8-10µg per tube or higher.

What RNA yields are expected with the PAXgene Blood RNA kit?

Typical yields of RNA isolated from 2.5 ml human whole blood are between 4 and 20 µg. However yield is highly donor-dependent, and in some cases higher or lower yields may be achieved. At least 95% of all samples with a WBC count between 4.8 and 11.0 x 106 cells/ml of blood will yield ≥3 µg of total RNA per tube. Currently we have data for storage at –20°C and –70°C for 96 months (In situ stability of RNA in blood specimens stored for 8 years (96 months) at –20°C and –70°C in PAXgene Blood RNA Tubes).

Can the PAXgene RNA stabilization solution/blood mixture from the PAXgene Blood RNA Tube be disposed of by adding bleach?

Yes. You may dispose of the waste product from the PAXgene Blood RNA Tube with a 5% sodium hypochlorite bleach solution, in a 1:9 ratio (1 part bleach : 9 parts tube contents).

What are possible reasons for blood draws with lower than expected blood volume?

Possible reasons for blood draws with lower than expected blood volume include: 1) The phlebotomist has not waited enough time for the blood to stop flowing into the tube. 2) The tube has "pushed back" from the needle end attached to the holder and the tube is no longer drawing. 3) Discard tube was not used. If used according to the instructions for use, there is no possibility of backflow during phlebotomy, and thus no possibility of reagent going into the patient’s arm.

Do the samples require pretreatment before starting the manual protocol or before loading onto the QIAcube instrument?
Yes. Samples have to be incubated at least 2 h at room temperature (18-22°C). Incubation of the PAXgene Blood RNA Tube overnight may increase yields slightly in some cases. Room temperature storage for up to three days has no effect on the RNA profile. During the preparation of the RNA the manual pretreatment consist of a centrifugation and a pellet resuspenion step (see the PAXgene Blood miRNA Handbook ).
